Quartet is seeking Family Nurse Practitioners or Psychiatric Mental Health Nurse Practitioners (PMHNP) licensed in the state of New Jersey to join our multidisciplinary team. The ideal candidate will be passionate about providing holistic care to those with serious mental illness, managing whole-person care and treatment for adult patients both virtually and in the home. This role involves serving as the "Quarterback" of the Care Team, providing longitudinal support of the patient care journey, developing individual care plans, and assessing patient progress. The candidate will also be responsible for conducting initial intake visits, prescribing and managing medications, and ensuring the longitudinal support of all patients. If you are an expert in diagnosis and treatment of patients with chronic and urgent medical/psychological conditions, have excellent communication skills, and are willing to travel locally at least 75% of the time, we encourage you to apply.
Responsibilities:
- Serve as the "Quarterback" of the Care Team, providing longitudinal support of the patient care journey, developing individual care plans, and assessing patient progress
- Provide expertise in general medicine and / or evidence-based behavioral health care to adult patients facing chronic and complex conditions both in the home and remotely
- Perform Initial Intake visits with patients, using assessment instrument criteria while engaging other interdisciplinary care team members
- Order, interpret, and analyze diagnostic tests. Recommend follow-up appointments and further treatment options
- Prescribe and manage medications and treatments as appropriate including the ability to execute injectables
- Capture diagnosis details, including billing codes, and submit claims
- Identify social needs and non-medical barriers to care for the patient and collaborate with the Care Team to address
- Manage psychosocial component of each patient's care
- Actively engage with members of the interdisciplinary team via frequent patient rounding to drive efficient care planning
- Initiate internal referrals to engage other expertise within the Quartet Care Team
- Develop a strong rapport with a diverse set of patients both in-person and via telehealth, supporting patient engagement in the program
- Uphold clinical customer service through the delivery of high quality, empathetic care
- Document concurrently all clinical findings and services provided in the electronic health record as required by compliance standards
