Reliance Hospice and Palliative Care is seeking a full-time or per diem hospice LVN in Orange County. The LVN will assist the RN in the planning and coordination of care with patients and family, as well as fostering communication between families, patients, and their care teams. We are looking for someone who has a demonstrated commitment to Reliance Hospice & Palliative Care philosophy of care, values, mission statement, and organizational culture. The ideal candidate must be conscientious with our clients, their families, and their caregivers, and demonstrate the ability to assess and respond to the needs of the patients and families in varied settings.
