About the Role:
We’re looking for a Spanish Bilingual Virtual Assistant with hands-on Chronic Care Management (CCM) experience to support a CCM provider in delivering consistent follow-up and quality care to patients with chronic conditions.
Key Responsibilities:
📞 Call patients and follow up on their chronic care needs.
💊 Review medications and assist with prescription-related processes.
📅 Assist with scheduling and patient follow-ups.
🚨 Review and address patient alerts.
📝 Document patient interactions, medication updates, and care-related information.
📋 Follow established CCM workflows and care plans.
🤝 Coordinate communication between patients and the healthcare provider when needed.
❤️ Provide compassionate and professional patient support.
📈 Help improve patient engagement, continuity of care, and the overall level of care provided.
Requirements:
- Bilingual English/Spanish with strong verbal and written communication skills.
- CCM experience required
- Experience with medications and prescription refills
- Healthcare or Medical VA experience
- Strong patient communication and documentation skills
- Ability to handle confidential patient information professionally
- eCareMD experience preferred
