Join our groundbreaking team and be part of the future of complex chronic care delivery. Uptiv Health is looking for a PRN Family Nurse Practitioner (FNP) to join our team in Troy, Michigan. As an FNP, you will provide clinical oversight, showcase highly developed nursing skills, bedside manner, customer service orientation, patient and provider education skills, as well as leadership skills. You will be responsible for onsite supervision of both clinical and administrative operations within Uptiv Health centers, preparation and administration of infusion medications and injection therapy, documentation of patient findings, and providing drug education to patients and caregivers. The ideal candidate will have active RN and FNP licenses in the state of Michigan, 2+ years of infusion experience, strong understanding of clinical processes and protocols, and exceptional ability to start Intravenous Therapy (IVs) and reconstitute medications. Responsibilities: - Provide onsite supervision of both clinical and administrative operations within Uptiv Health centers; inclusive of policy improvement and operationalization - Prepare, document, dispense, and administer infusion medications and injection therapy in accordance with the drug package insert (PI), internal policies, and insurance authorizations - Meticulously document findings from patient discussions, visit notes, vital signs, and patient health histories in our Electronic Health Record - Provide generalized drug education (i.e., side effects as appropriate) to patients and caregivers - Serve as a reliable, clinical point of contact for referring providers on clinical questions and requests such as: updated order requests, order clarification, patient related questions, and drug related questions
