Requirements and responsibilities

Job Overview We are seeking a skilled and patient-focused Nurse Practitioner (FNP) to provide primary care services via telehealth. This role involves conducting virtual consultations, managing chronic conditions, and delivering high-quality care to Medicare patient populations. Key Responsibilities • Provide primary care through telehealth (video consultations) • Assess, diagnose, and manage acute and chronic conditions • Order and interpret diagnostic tests and lab results • Develop and manage individualized treatment plans • Educate patients on disease management and preventive care • Collaborate with physicians and interdisciplinary care teams • Maintain accurate and timely documentation in EMR Required Qualifications (Must Have) • Active New York Nurse Practitioner (FNP) license • Experience with eClinicalWorks (ECW) • Active Medicare and Medicaid provider numbers Preferred Qualifications • Primary care experience • Telehealth experience • Experience working with geriatric patients • Strong communication and patient engagement skills
