We know this is vital in realizing our shared mission to improve speed to quality mental health care for all. innovaTel is looking for a full-time Licensed Clinical Social Worker licensed in Minnesota to work with patients across the LIFESPAN in an Outpatient Community Mental Health Center Setting. The successful candidate will be responsible for performing initial mental health diagnostic assessments, developing individualized treatment plans, and documenting clinical findings and services provided in the electronic medical record. They will also participate in treatment meetings, consultation, and collaboration with members of the clinical team. The ideal candidate should have relevant and active LCSW license in the state of Minnesota, be certified and/or experienced in common therapeutic modalities, and have excellent communication and critical analysis skills. Preferred qualifications include experience in caring for patients with Serious Mental Illness and co-occurring substance use disorders, as well as previous telemedicine experience and proficiency in using online tools and technology to deliver care and communicate with patients. Responsibilities: - Perform initial mental health diagnostic assessments with treatment recommendations. - Develop an individualized, prescriptive treatment plan to fit the needs of each patient using appropriate diagnostic criteria. - Develop a strong rapport with a diverse set of patients exclusively through the telehealth modality. - Document concurrently all clinical findings and services provided in the electronic medical record as required by standards of care. - Participate in treatment meetings, consultation, and collaboration with members of clinical team. - Be present and available on-screen for the duration of the contracted hours.
