Physician's Choice Home Health Care seeks a dedicated Physical Therapist (PT) to join our team. As a Physical Therapist, you will play a vital role in helping our patients regain their independence, mobility, and overall well-being. Responsibilities: - Conduct thorough assessments of patients' physical conditions and needs. - Develop and implement individualized treatment plans tailored to each patient's goals. - Administer physical therapy interventions, exercises, and modalities. - Monitor and document patients' progress, adjusting treatment plans as necessary. - Educate patients and their families on exercises and techniques for at-home care. - Collaborate with a multidisciplinary team of healthcare professionals. - Maintain accurate and up-to-date patient records in accordance with state regulations. Qualifications: - Current Oregon state license as a Physical Therapist. - Graduation from an accredited physical therapy program. - Excellent communication and interpersonal skills. - Ability to work independently and as part of a team. - Strong organizational and documentation skills. - Experience in home health or a related setting is a plus.
