Teladoc Health is seeking a part-time Physician Case Manager to assist members in navigating their medical journey. The ideal candidate is a physician who thrives in a patient advocate role and is available to commit to 15-20 hours per week. The role involves obtaining comprehensive health history, directing patients to appropriate service lines, working in partnership with the clinical team, and maintaining regular contact with members. The candidate must be board certified in a specialty/subspecialty, possess excellent communication skills, and demonstrate effective organization and time management skills. Responsibilities: - Obtaining a comprehensive health history by leading members through a systematic and dynamic intake assessment - Directing patients to the most appropriate service line based on their medical condition - Working in partnership with the clinical team in support of case development - Supporting expediting procurement of patient medical records - Writing comprehensive clinical summary and presenting expert physician findings - Communicating with patients in a caring and compassionate manner - Tracking all communication with members in Salesforce - Adhering to and abiding by all regulations, policies, work procedures, and instructions
