NYPCC is seeking a positive, motivated, compassionate, organized, and detail-oriented Bilingual Case Manager to join our team. The ideal candidate will work well with a population with pervasive mental health issues and will be responsible for following up with daily incoming referrals, scheduling intake appointments, assignments of cases, and following up with multiple external agencies. This is an exciting opportunity to be part of a leading pioneer of community mental health services and contribute to the well-being of our clients and the community. Responsibilities: - Process all intake referrals on a daily basis. - Gather all pertinent information to process the referral including previous history of psychiatric or mental health treatment, assess for any developmental delays, explore academic or work-related difficulties, and review criminal history. - Review and check sex offender registry. - Access and download Psyckes information. - Enter the referrals in electronic health record and scan all necessary documents into system. - Schedule intake appointments in electronic health record system. - Verify Medicaid eligibility to ensure that a potential client has proper medical coverage. - Collaborate with referral sources to provide updates on the status of referrals. - Provide outside resources as appropriate for non-admit or clients that require an alternative level of care. - Obtain and review foster care referral packages and hospital packets prior to scheduling an intake appointment. - Discuss and review hospital referrals with administration prior to scheduling. - Follow up with assigned case assistance to ensure timely/appropriate reassignment. - Track assigned referrals in IMA. - Provide assistance to therapists as needed with referrals for developmental testing or other appropriate service providers. - Conduct follow-up to individuals that fail to attend intake appointment. - Assist clients with eligibility problems and refer out as needed. - Perform other duties as assigned by Supervisor.
