Requirements and responsibilities
Job Title: Family Nurse Practitioner (Float)
Duration: 5 plus month with possible extension
Location: Orange Park, FL
Pay Range: $75-$80.00/hr
Must be willing to float to cover Macclenny, Callahan, and Jacksonville surrounding area. 2-3 weekdays and 6/12 weekend shifts (sat/sun).
Appx 30-38 hours a week. This position will require possible holiday rotations.
Nurse Practitioners will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients.
The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients.
This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning. This role will report to the practice manager.
Provide Primary and Family evidenced-based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients.
Encourage patients to become active participants in their own healthcare and well-being through services such as health screenings, behavioral health and wellness, chronic condition management
Master's Degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse role.
