Through our mobile app, patients engage in regular video visits along with text messaging, photo journaling, and personalized educational content. We're deeply invested in our core value of putting patients first, and equally committed to building a culture where every employee is valued and we learn and grow together.About the Role:The Care Delivery team builds the tools that let our clinical team run a high-performing virtual practice — from prescribing and prior authorizations to the day-to-day workflows clinicians rely on to do their best work with patients. This role is highly visible within Form Health: the tools you ship directly shape how our clinical team practices and, downstream, how our patients experience care. We're looking for someone who's genuinely energized by hard, regulated, multi-stakeholder problems — with clinical, patient, payer, and operational constraints — as well as someone who uses AI as a real part of supercharging their work, rather than a buzzword on a resume.This role will report to our Lead Product Manager for Care Delivery, with significant ownership from day one and plenty of room to help shape how the practice scales as the team grows.
