We are also deeply committed to building more sustainable and durable payment models for this care – models which reward clinicians for delivering the right care, in the right place at the right time. OPPORTUNITYCVAUSA is seeking a Value Based Care Transformation Specialist to join our Novocardia Division. The Value Based Care Transformation Specialist will function as a critical member of Novocardia’s value-based care (VBC) team. The role will collaborate with our practices and partners to implement and optimize clinical care model and manage performance on VBC contractual metrics to drive success in our VBC arrangements. This individual will be responsible for working with practices across the CVAUSA network and will devote at least half of his/her time to working with practices in Florida.Job Responsibilities:Collaborate with clinical practices to implement clinical programs designed to improve the quality and value of cardiovascular care and iteratively improve the operational performance and efficiency of these programs. Specific related responsibilities include:Monitoring contractual and quality metrics, setting metric goals, and creating mitigation plans to improve contractual performance when these goals are at risk.Training clinicians and practice staff on how to execute against care model operational processes.Continuous assessment and improvement of care model programs operational processes in collaboration with Novocardia leadership and the practice staff.Lead recurring clinical quality meetings with practice leadership to review clinical and operational outcomes, collaborate on unstable and high risk-patient outliers, and create and execute quality improvement plans.Close collaboration with wrap around care management team director including ensuring alignment in goals and outcomes.Partner with data and analytics teams to design and manage clinical outcomes and dashboards.Partner with Account Management on tracking and achieving payor-facing contractual metrics.Manage priorities and strategies for clinical programs, quality measures and other patient-centered value programs adhering to industry standards, benchmarks (e.g., HEDIS, Stars), and contract requirements to drive success across the company’s portfolio of contract types and population (Medicare, Medicaid, Commercial).Collaborate with the senior leadership team to develop, communicate, and execute on Novocardia’s strategic vision.Coordination among product, operations and clinical to ensure seamless workflow and clear deliverablesIdentify in market barriers and work with clinical services director to find new solutions, iterate on clinical workflows and improve qualityQualifications and Background:Bachelor’s degree, nursing, healthcare administration or other clinical degreeMinimum of 3 years of experience in value-based care operationsBackground in value-based care clinical models including knowledge of quality metrics, regulatory environment and best practices requiredExperience working with practices and providers to drive success in value-based care arrangements requiredExperience in delivering and managing the quality and process improvement cycleBackground in interpreting and revising data dashboards to ensure optimal patient outcomesSignificant experience with developing, tracking, and driving performance against key performance indicators in clinical delivery settings.Experience in clinical product organizations with a proven track record of developing and implementing new clinical productOutstanding organizational and written and verbal communication abilities.Ability to spend a significant amount of time traveling to clinical practices.Change management experience highly preferredJob Location:  Remote, must reside in FL, travel 25-50%
