About the roleThe Patient Intake Coordinator serves as a vital member of our interdisciplinary care team, supporting patients across the continuum of both brain health and mental health services. This role ensures patients receive compassionate, timely, and clinically sound care by assisting with screening, safety assessments, and treatment coordination. Working closely with clinicians and care teams, the Patient Intake Coordinator helps facilitate seamless patient experiences from initial contact through ongoing treatment and follow-up.This position is ideal for an individual who combines strong patient-facing skills with empathy, flexibility, and adaptability to support patients in a fast-paced, evolving clinical setting.Key ResponsibilitiesPatient Intake & EngagementConduct patient screening calls for individuals presenting with mental health and/or brain health concerns, following structured protocols and applying sound judgment to assess needs.Serve as the first point of contact for individuals making initial inquiries into Salma Health, assisting with intake form completion and onboarding processes.Support patients in understanding and navigating insurance coverage, confirming eligibility, and facilitating access to care.Document all patient interactions accurately and promptly in designated dashboards and the electronic health record (EHR).Identify potential safety risks and escalate concerns in accordance with organizational risk management procedures.Collaborate with physicians, therapists, and care coordinators to ensure patients are connected to the appropriate care pathways.Respond to patient inquiries via phone, secure messaging, and email, while managing outbound calls for engagement and follow-up.Care Coordination & NavigationSupport treatment planning and discharge processes with clinical teams to ensure timely, coordinated services.Work in collaboration with the local provider partnerships team, making outbound calls as a connector between external partnership opportunities and Salma Health.Schedule intake and follow-up appointments, coordinating logistics based on clinical recommendations of the patient’s care team and patient needs.Coordinate appointment rescheduling and manage related administrative workflows and key tasks.Support & Risk ManagementMaintain compliance with clinical, legal, and ethical documentation standards.Participate in supervision, team meetings, and professional development activities to maintain high standards of care and alignment with best practices.Promote a patient-centered approach that fosters trust, compassion, and clear communication throughout the care journey.Serve as the liaison between levels of care, ensuring continuity and appropriate transfer of clinical documentation.Additional cross-functional responsibilities as needed, including support across Commercial Operations.Required QualificationsCompliance with all state board requirements, including fingerprinting and application submission.Proficiency with telehealth platforms and remote clinical workflows.Strong interpersonal, communication, and documentation skills.Experience working in a mental health clinic, psychiatric setting, or medical office, with a strong understanding of patient workflows and multidisciplinary collaboration.Experience working in a fast-paced call center environment where multi-channel lead capture requires immediate follow-up to maintain optimal clinic capacity.Ability to work independently while contributing effectively to a collaborative, interdisciplinary team environment.Compensation & BenefitsHourly Rate: $20-$26 per hour for 40 hours per week (based on factors such as geography, relevant experience and skills).Work Location: Remote on PST Zone (with requirement to be on-site twice per year)Schedule and Shift Structure: Patient Intake Coordinators can choose one of three shifts to ensure coverage from 8:00 AM to 8:00 PM PST, Monday through Sunday (shift availability depends on staffing needs).All coordinators work 5 days per week, 40 hours per week with one hour lunch break with 2 consecutive days off.Shift A — 8:00 AM to 5:00 PM PST; Shift B — 10:00 AM to 7:00 PM PST; Shift C — 11:00 AM to 8:00 PM PSTWeekend availability is required. Days off rotate across the week on a set schedule.
