Requirements and responsibilities
Job Title: MTM Pharmacist (Part Time)
Duration: 12 plus month with possible extension
Location: Fully Remote
Pay Range: $57- $60.00/hr
The pharmacist will be responsible for medication therapy management (MTM) services, including outbound calls for comprehensive medication reviews (CMR) and delivering in-depth consultations on statin medication use per AHA guidelines.
The pharmacist will transcribe patient-specific prescription information, diagnosis information, and recommendations into a third-party, web-based application. The data entered by the pharmacist will be used to create a standardized document that will be mailed to individual patients.
Experience Requirements
● At least 4 years of experience as clinical MTM pharmacist. (Management experience preferred)
● Familiar with driving Part D Star measures. Specifically, SUPD, SPC, and CMR.
● Understanding of pharmaceutical and medical vocabulary
● Clinical experience in medication review.
Essential Job Responsibilities:
● Conducting outbound phone calls. (100 calls per day)
● Completing comprehensive medication reviews.
● Conducting medication therapy management services.
● Accuracy and speed in entry of patient-specific prescription information, diagnosis information, and recommendations into a third-party, web-based application
