We’re rapidly expanding and building a national virtual care team to support patients across all 50 states.Schedule: Full-Time, Monday-FridayWe’re looking for an experienced Virtual Care Coordinator to join our growing team. In this role, you’ll support patients and clinicians across the country by managing virtual patient check-ins, handling inbox messages, and ensuring seamless care coordination in a fully remote environment. This is an ideal opportunity for someone with prior virtual care experience who is comfortable navigating EMRs and multitasking in a fast-paced, digital-first clinic in a health tech startup environment.This role reports to the Nationwide Virtual Care Practice Manager.Responsibilities:Conduct remote patient check-ins and pre-visit preparation including medication reconciliation, health history intake, and screening questionnaires Document medical information accurately in the EMR and prep charts ahead of clinician visits Communicate with patients regarding upcoming appointments, test results, and follow-up instructions Support clinicians during virtual visits by entering real-time documentation, assisting with orders, and coordinating next steps Coordinate care: manage referrals, prior authorizations, and medication refill requests Ensure prompt and professional communication with patients, clinicians, and care team members Collaborate with the virtual care team to identify and improve workflows Maintain a warm, inclusive, and professional patient experience aligned with knownwell’s values Requirements: High school diploma or equivalent 2+ years of experience in a virtual care, patient coordination, or healthcare support role Familiarity with virtual care workflows in a primary care or chronic care setting Prior experience supporting care in a multi-state, remote care delivery model Strong communication and organizational skills Comfort with remote tools, secure messaging, EMR platforms (Athena preferred) Tech-savvy, adaptable, and comfortable operating independently in a remote setting Knowledge of obesity medicine, weight management, or chronic care workflows (preferred)Familiarity with HIPAA-compliant telehealth platforms (preferred)Experience with Athena EHR (preferred)Pay & Perks:💻 Fully remote opportunity🩺 Medical, dental, and vision insurance📈 401K retirement plan with company match🏝️ Up to 20 days of PTO per year + company holidays👶 Up to 14 weeks of parental leave (12 for non-birthing parents) 🏡 Annual work from home stipend for remote employees
