JOB OVERVIEW:Cardiovascular Services of America (CVAUSA) is the largest private and independent cardiology network in the United States. Our inclusive and diverse network brings together top cardiovascular specialists and thought leaders who offer regional perspectives and a broad strategic vision for the future of patient care.ESSENTIAL DUTIES AND RESPONSIBILITIES (included but not limited to):We are looking for candidates who want to use their expertise to improve health, wellness and care access for patients with chronic conditions. We are recruiting candidates that have experience with cardiology patients, able to develop purposeful relationships with patients, and can provide clear reporting to the cardiology care team. The CCM assistant has the following responsibilities:Develops a patient coaching relationship that supports the patient care plan and quality of life for the patientIt is a trusted and empathic support for patients and families dealing with chronic conditionsCommunicating abnormal findings to the cardiology care team.Monitors and encourages patient adherence to care plans and participation Effectively communicates findings to the care teamSupports new patients in the enrollment processWorks as a liaison for the patient with the practice and providersIdentifies and escalate changes to providers to reduce events such as hospitalizations or urgent care visitsWorks effectively with the EMR and CCM platformBasic monitor troubleshooting with patients for the CCM platformAbility to follow clinical protocols.Participate in continued team education through shared learnings.Contribute to a high-quality service both to physicians and patients.Stays current and complies with state and federal regulations/statutes and company policies that impact the employee's area of responsibility.JOB REQUIREMENTS AND QUALIFICATIONSHigh School diploma or GED1 year of experience in remote chronic care managementExperience with cardiology patients - specifically heart failureRegistered Nurse (RN)Medical Assistant (MA)Certified Nurse Assistant (CNA)Experience with EMRs – specifically Athena, NexGen, GEMMS and e-clinical worksGreat communication and multi-tasking skillsSCHEDULEShift options are:8-hour shiftsActual hours are based on customer/patient time zoneBENEFITS401(k)Dental insuranceHealth insuranceLife insurancePaid time offVision insuranceMedical specialties:CardiologyPay is dependent on experience with CCM, experience with cardiology patients and language skills $20.00 - $24.00 per hourCVAUSA is an Equal Opportunity Employer
