Quartet is seeking a Medical Director to join their collaborative, multidisciplinary care teams in the Whole Health program. The role involves providing coordinated, patient-centered care for a panel of patients, clinical supervision, and management responsibilities. The ideal candidate will have a medical doctor (M.D.) or Doctor of Osteopathy (D.O.) degree, active Internal Medicine or Family Medicine Board Certification, and 3+ years of clinical experience in providing primary care. Residency in the greater New Jersey area and knowledge of the New Jersey market is preferred. The candidate should also have experience in value-based care models and a passion for caring for individuals with complex chronic medical, behavioral, and social needs in a multidisciplinary, integrated, or collaborative model.
Responsibilities:
- Provide oversight of the care team to ensure exceptional, evidence-based, and patient-centered care
- Serve as the primary clinician for a small panel of high-risk, complex patients
- Perform home and community-based follow-up and urgent care visits, as well as virtual visits
- Facilitate multidisciplinary case rounds and create appropriate patient-centered care plans
- Collaborate with external providers and community-based organizations to advocate for patient care aligned with their goals
- Ensure timely and appropriate documentation within the electronic medical record and systems that support care for patients
- Ensure timely, accurate, and appropriate documentation for APC visits and visits by other clinicians
- Perform 1:1 in-person observations as well as case and chart reviews on a regular basis
- Coach and train advanced practice clinicians in clinical care, workflows, EHR documentation, and multidisciplinary team collaboration
- Participate in workflow, policies & procedures improvement and implementation
- Participate in interviewing, hiring, and training of clinicians and members of the care team
