We are champions for better health, for those who need us most.Position Summary:The Pharmacy Technician will be responsible for addressing pharmacy referrals and assisting members with medication access, education, and adherence to support kidney health management. The Pharmacy Technician will work collaboratively with clinical and care management teams to identify medication opportunities and resolve medication safety concerns associated with transitions of care and chronic disease management. Daily activities will include accurate data collection and data entry in HealthMap’s documentation system, communicating with HealthMap’s interdisciplinary care team, and maintaining individual metrics. The Pharmacy Technician will also create and maintain medication access resources and participate in pharmacy-driven educational initiatives designed to improve member outcomes and support member engagement efforts.Responsibilities:Assist with pharmacy referrals related to medication access, affordability, education, adherence, optimization, and safety with pharmacist oversightReceive and respond to member’s medication-related questions in a timely mannerSupport medication reviews by gathering, reviewing, and documenting medication information for clinical evaluationCollaborate with nurses, pharmacists, care coordinators, providers, and other interdisciplinary care team members to support member care plans and transitions of carePerform accurate data entry and maintain timely, complete, and compliant documentation within designated systemsDevelop, maintain, and update medication access and pharmacy resource materials for internal teams and membersMonitor assigned tasks and referrals to ensure timely follow-up and resolutionAdhere to policies, procedures and state and federal regulations to ensure compliance and confidentiality of Protected Health Information (PHI).
