Position: Case Manager RN
Rate: $40/hr.
Location: Remote in Michigan.
The Case Manager RN coordinates a multidisciplinary, person-centered care management program for health plan members with complex medical, behavioral, and social needs. This role serves as the primary point of contact for members, caregivers, and providers through telephonic and digital communication. The Case Manager RN assesses member needs, develops and implements individualized care plans, coordinates community and healthcare resources, supports care transitions, delivers health education, and monitors outcomes to improve overall health and continuity of care.
Requirements
* Active, unrestricted Michigan RN license.
* Nursing Diploma or Associate Degree required; BSN preferred.
* Minimum of 3 years of clinical nursing experience.
* Case management experience in a managed care setting preferred.
