www.myisaachealth.comAbout the RoleWe’re looking for a Care Navigator to join our existing care team who has prior experience coordinating/navigating care for patients, caregivers, and their loved ones - bonus points if you’ve worked with a patient population who experience cognitive challenges. This role requires genuine empathy, patience, exceptional time management and communication skills. You will spend >70% of your day on the phone connecting with and coordinating care for your patients and their caregivers.
