Requirements and responsibilities


Job Title: Field Case Manager/Behavioral Health Case Manager - Tampa FL. Duration: 3 plus months with possible extension. Location: Tampa FL (Fully Remote). Pay Range: $34.00 - $37.00/hr. Work Environment: - Work from Home. - Candidates must reside in East Tampa, FL. - Training will be conducted remotely via Microsoft Teams. - Candidate will travel approximately 75% of the time within the region seeing members at home, in assisted living facilities, and nursing homes. Language Requirement: - Fluent in Spanish/English (reading, writing, speaking). Responsibilities: - Telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness. - Effectively manage a caseload that includes supportive and medically complex members. - Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member’s overall wellness through integration. - Utilizes case management and quality management processes in compliance with regulatory and accreditation guidelines and company policies and procedures. Required Experience: - One year Case management experience. Preferred Experience: - Case Management Certificate. - Long term care experience. Education: - Bachelor's degree required. - No nurses. - Social work degree or related field preferred. Schedule: - Monday-Friday 8am-5pm.
