This isn’t just a job—it’s a chance to help caregivers feel confident, informed, and supported as they begin a journey that could transform their caregiving experience.Why This Role MattersCaregivers of people living with dementia often feel overwhelmed and under-supported. Ceresti’s GUIDE program offers personalized coaching, care coordination, care planning, respite care, and resources—at no cost to eligible families.As a Program Coordinator, you’ll ensure that every patient and their caregiver moves through the onboarding process with clarity, care, and confidence. You’ll monitor assessments, track progress, coordinate care and prepare families for their first connection with their dedicated coach.What You’ll DoGuide patient and their caregivers through the onboarding process after enrollmentMonitor completion of dementia screening and health assessmentsCoordinate tablet shipment and provide orientation to the GUIDE programSchedule and track formal neurology evaluations with our clinical partnersReview documentation and prepare applications for CMS submissionTrack progress and report status updates to internal teamsEnsure a warm handoff to Engagement Specialists and CoachesCollaborate on software workflowsAdapt to evolving processes and contribute to continuous improvementThe Right FitYou’re a great match if you:Are highly organized and detail-orientedCommunicate clearly and compassionatelyThrive in a dynamic, mission-driven environmentCan adjust quickly to changes in workflows and prioritiesUnderstand (or are eager to learn about) dementia care and caregiver needsEnjoy working cross-functionally with clinical and tech teamsQualificationsBachelor’s degree in healthcare administration, social work, public health, or related field preferredExperience in patient navigation, care coordination, or healthcare customer serviceStrong interpersonal and communication skillsDetail-oriented with excellent organizational and time management abilitiesComfortable working in a fast-paced, tech-enabled environmentDemonstrates flexibility in response to changes in workflows, tools, and program requirementsFamiliarity with CMS programs or dementia care is a plusWorking ConditionsRemote work environment with flexible scheduling options across time zonesStandard business hours with potential for some evening or weekend availabilityRequires dedicated, private workspace suitable for confidential consultations
