The Opportunity: The Medical Coder – Home Health & Hospice is responsible for accurate coding and documentation review to support compliant billing and reimbursement processes. This role primarily focuses on Medicare coding for home health episodes, ensuring all clinical documentation aligns with regulatory requirements and supports appropriate reimbursement under the PDGM model. The position works closely with clinical and administrative teams to maintain coding accuracy and efficiency.Key ResponsibilitiesPerform accurate and timely coding for Home Health episodes, including Start of Care (SOC), Resumption of Care (ROC), and Recertification (RC).Apply appropriate ICD-10 codes in accordance with Medicare guidelines and agency policies.Review clinical documentation to ensure completeness, accuracy, and compliance with regulatory standards.Validate diagnosis coding, including sequencing and selection of primary and secondary diagnoses.Ensure accurate application of PDGM grouping, including assessment of diagnoses and comorbidities.Review intake and discharge charts for coding accuracy and completeness.Prepare and fax discharge summaries on a weekly basis (every Monday).Collaborate with clinicians and intake teams to clarify documentation and ensure coding integrity.Maintain productivity and quality standards for coding turnaround times.Qualifications RequiredActive coding certification is required (e.g., CPC, CCS, HCS-D, or equivalent).Proven experience in Home Health and/or Hospice coding, with strong knowledge of Medicare guidelines.Hands-on experience with PDGM grouping model, including diagnosis selection and comorbidity adjustments (non-negotiable requirement).Strong understanding of ICD-10-CM coding conventions and regulatory compliance.Experience reviewing clinical documentation for accuracy and completeness.High attention to detail and ability to work independently in a fast-paced environment.Effective communication skills for collaboration with clinical and administrative teams.Service Fee: Industry standards applicable to the state depending on work experience and level of expertise.Hours: US Eastern Standard Time (EST) – 9am to 6pmWhy Limitlessli?
