As a Nurse Care Manager, you'll support Medicare-age members virtually through proactive check-ins, remote patient monitoring (RPM), chronic care management (CCM), review, triage, care coordination, and education. You'll work alongside our Nurse Practitioners, physician leaders, and care team to deliver a concierge-level experience that helps patients stay healthy, independent, and ahead of problems. This is a fully remote position with consistent weekday hours (Monday–Friday, ET).
